Payer contracting is local. Your Blue Cross Blue Shield plan is a specific licensee tied to your state, national carriers structure their networks market by market, and while the BlueCard program lets a Blue member's claim cross state lines, the contract you sign is always with the payers operating where you are. This is a plain-English map of that landscape, state by state.

Northeast

Independent Blues, Anthem-brand plans, and a distinctive set of New England regional payers.

Southeast

Dominant state Blues, large Medicare Advantage markets, and regional rental networks.

Midwest

A mix of independent Blues, Anthem states, and the multi-state HCSC and Wellmark plans.

Southwest

Large metro markets served by the HCSC family of Blues and the national carriers.

West

Two-Blue states, Regence and Premera Blue plans, and the West's large integrated health systems.
