The Optum Contract: What UHC-Facing Facilities Should Know

For UnitedHealthcare members, behavioral health runs through Optum — not UHC. Here's what that means for which entity you contract with, which portal runs your credentialing, and the leverage of one of the two dominant national BH networks.

If your facility serves UnitedHealthcare members, there's a good chance the contract you actually need isn't with UnitedHealthcare at all. It's with Optum. That single fact trips up more behavioral health facilities than almost anything else in payer contracting — and getting it right is the difference between months of productive work and months spent knocking on the wrong door.

Optum is UnitedHealthcare's behavioral health network

UnitedHealthcare's behavioral health is administered by Optum — a fellow UnitedHealth Group company. Optum was historically known as United Behavioral Health (UBH) and today operates as Optum Behavioral Health. The two are corporate affiliates: UnitedHealthcare engages its sibling company as the behavioral health manager.

The practical translation is blunt. A facility contracting for UnitedHealthcare members' behavioral health services contracts with Optum, not UnitedHealthcare. "Getting in-network with UHC" for behavioral health is, in practice, getting in-network with Optum. If you've been aiming at UnitedHealthcare's medical contracting channels, you've been aiming at the wrong entity.

Provider Express is the front door

The tell is the portal. Optum's behavioral health credentialing and provider tools run through Provider Express — Optum's behavioral health provider portal — not UnitedHealthcare's main provider site.

This is where facilities lose time. They start on UnitedHealthcare's medical provider portal, work through channels that were never built for behavioral health, and wonder why nothing moves. The behavioral health path runs through Provider Express because that's where Optum administers it. Knowing that on day one saves a lot of wasted motion. When you set out to join UHC's behavioral health network, the question isn't "how do I reach UnitedHealthcare?" — it's "how do I get through Optum's front door?"

One of two networks that dominate national behavioral health

Optum isn't just one carve-out among many. Optum and Carelon (Elevance's behavioral health arm) are the two largest behavioral-health-specific networks in the country. For most facilities, they are the highest-leverage contracts to hold — and, by the same token, the ones where the leverage sits most heavily on the payer's side.

That matters because it frames the negotiation. You're not dealing with a boutique network that needs your beds to fill a gap. You're dealing with one of the two entities that define national behavioral health contracting, with a correspondingly wide view of the market and a correspondingly strong hand.

What the leverage asymmetry means for you

A large, dominant network knows its own rates across the whole market. You, walking in, historically knew only your own contract. That asymmetry is exactly the problem behavioral health rate intelligence exists to close: the only way to meet a counterparty who can see the whole distribution is to arrive with a defensible read on where your rate actually stands.

Against a network of Optum's scale, an ask built on a vague sense of "we should get more" doesn't survive contact. An ask grounded in actual, comparable, confidence-scored peer data is a different conversation — one where you're negotiating from evidence rather than hope.

Before you prepare anything else for a UnitedHealthcare-facing behavioral health negotiation, nail down three things:

  • Which entity you're contracting with — Optum, not UnitedHealthcare.
  • Which portal runs your credentialing — Provider Express.
  • Which network you're joining — one of the two that dominate national behavioral health, with the leverage that implies.

Get those right and everything downstream is aimed correctly. Get them wrong and the most careful preparation lands on the wrong desk.
